How do I submit a Health Sciences project or Qualtrics consultation request?

To request support from Learning Technologies & Innovation for Health Sciences course design projects or consultation on Qualtrics survey construction, submit the appropriate form. The steps outlined below feature screenshots from the Health Sciences project intake form, though the same general steps apply to the Qualtrics consultation form.

Learning Technologies & Innovation utilizes a tool called Asana to manage these requests. The first time you submit a project request, you will be prompted to follow the steps below to verify your Loyola email with Asana. Doing so will allow you to communicate with our staff about the details and status of your request. Subsequent form submissions will follow a similar process whereby you are emailed a "magic link" from Asana.

Access the form.

Access the appropriate form for your request: Health Sciences Innovation Project Requests or Qualtrics Consultation Request.

Enter email.

  1. Enter your email address ending in @luc.edu in the Email address field.
  2. Then, select Verify with email.

Note: You must enter an email addressing ending with @luc.edu to gain access to the form.

Select Continue.

After verifying that your email address is entered correctly, select Continue to proceed.

Locate the email verification message.

Select Verify email

In your LUC Outlook account, locate a message with the subject line Action Required: Reserve your Asana account. Select the Verify email button.

Note: The email may take several minutes to arrive. If cannot locate the email in your inbox or junk folders, you will find the option in Asana to request a new verification email.

Complete the form.

Enter the details of your request:

  1. In the Requesting School field, note which Loyola school or college you are primarily affiliated with (e.g., Parkinson School of Public Health).
  2. Use the dropdown menu to choose which Project Category most closely matches your request.
  3. Type a detailed Request Description to provide details about the project request.
  4. (Optional) Use the calendar-picker to supply a due date for the requested project.
  5. Select Submit to submit your project request to Learning Technologies & Innovation staff.

Note that your Loyola email address is pre-filled in the form. You can correspond with Learning Technologies & Innovation staff using this email address.

Upon submitting the form, you will receive a confirmation email containing the details of your request. To ask questions or to add details to your project or consultation request, reply to the confirmation email.
